Medicare - Chronic Disease Management Plan

Some families may be eligible for Medicare rebates through a Chronic Disease Management (CDM) Plan. Eligibility is determined by your child's GP based on their individual health and developmental needs.

As of 1 January 2026, Medicare provides a rebate of $61.80 per session for up to five allied health sessions per calendar year.

How the process works

  • 2Provide the referral paperwork to Raina Griffin Speech Pathology prior to or at your child's appointment.
  • 3Full payment for the session is made on the day of therapy.
  • 4After the appointment, your receipt can be uploaded through the Medicare app or submitted via your MyGov/Medicare online account.
  • 5Medicare rebates are typically processed within 1-3 business days, depending on your bank and Medicare processing times.
